Schedule Your Exit Interview

Please reference the business card you were given the last night of your JADE class. This is your assigned case manager. You must contact your case manager either by email or phone to schedule your exit interview (a list of case managers and their contact information is provided below). If you do not see your counselor's name in the list below or you misplaced the business card and are not sure who you were assigned to, contact "Unknown" and you will be directed to our Administrative Assistant.
