This form MUST be filled out and submitted to confirm your attendance and secure your seat for the class.

My child is aware of the strict dress code and will dress appropriately.

I understand a parent or legal guardian must attend and stay with my child the entire program.

I understand there is no food provided or available for purchase (please eat dinner before attending).

I understand there is no child care and all siblings or young children not enrolled will not be admitted.

If I need to reschedule, I will be charged a rescheduling fee of $50 if I do not call 72 hours (3 business days) prior to the first day of my scheduled class.
